  • Proof of Income for all household members (dated within the last 30 days of submission):
    • Copies of paycheck stubs (if employed)
    • Weekly – 6 current consecutive pay stubs
    • Bi-Weekly – 3 current consecutive pay stubs
    • Semi-Monthly – 4 current consecutive pay stubs
    • Monthly – 2 current consecutive pay stubs
    • New-Hire – Letter on company letterhead containing ALL: 1. Start Date,
    • 2. Hourly Rate, 3. Approximate Average Hours and 4. Frequency of Pay)
    • Copies of benefit letters for Social Security, SSI, SSDI etc.
    • Copies of all other income documentation, i.e. child support, pensions, etc.
    • Self-employment – IRS Tax Forms 1099 or Schedule C (if applicable)
  • Proof of Assets (dated within last 30 days)
    • Real Estate – mortgage, or deed
    • Life insurance policies, stocks, bonds, trust, other Investments
    • Most recent bank statement
  • Financial aid, education grants & scholarships for any household member attending college/ technical school (if applicable)
  • Receipts for out-of-pocket medical or disability expenses ONLY IF
    • The head of household, spouse or co-head is 62+ years of age
    • Or the head of household is a person with disabilities
  • Proof of allowable deductions for head of household, elderly disabled or handicapped medical, child care, students 18 years or older.
  • Letter on letterhead from child care provider and/or receipts for out-of-pocket child care expenses (if applicable).

No, recipients of Housing Assistance Payments via the voucher program must be active participants of the Housing Choice Voucher Program, and both participants and voucher-subsidized units must be in good standing with program requirements.
No, this is not a loan and does not have to be repaid. The assistance is provided on behalf of eligible families and individuals via federal funding distributed by the U.S. Department of Housing and Urban Development (HUD).
Applicants are NOT eligible for additional rental assistance if they are receiving rental assistance from another agency.

No. CMHA will make the payments directly to the Landlord. The Landlord must be willing to accept payment from CMHA on your behalf.
No, the Housing Choice Voucher Program is for rental housing only.
To ensure funding is distributed equitably, CMHA will select applications through multiple mini lotteries from the preliminary application database.
If you applied for Housing Choice Voucher programs in Cuyahoga County in previous years, you need to reapply. Applicants will not be considered for this specific program unless they submit a preliminary application through cmha.net/apply.
CMHA will select preliminary applicants by lottery as funding becomes available. Because of the random selection, your name could come up within a few days, or it could take months. Your preliminary application is valid for 18 months. If it expires, you can immediately apply again to stay in the selection pool.
CMHA will notify selected preliminary applicants by email. Having an active private email account is necessary for the preliminary application process. Please keep your contact information (like phone, email address and mailing address) as well as household income and composition up to date through the preliminary application portal so we can contact you if and when your name is selected.
